Onces you’ve gone through the checkout process you can use the forms below to save time in our office and get information to us from your previous providers. If you have any questions please give us a call. Or use the chat feature in the bottom right corner.
PWC Registration Form – this form is used when there are multiple people under your subscription. Please fill it out to let us know who they are.
PWC New Patient Medical Information Form – this form is to let us know your medical information. There is duplicate information between this form and the registration form which we apologize for but we need the information filled in on both forms.
Employer Covered Sign-up Form for Employees – this form is for the employees of a company that has signed up for Peak Wellness Center The form lets us know who is covered under your membership through your employer.
Release of Information Request Form – this form is used when you need to request that your medical information be sent to us from your previous provider.
